A simple, elegant hypothesis

I have been a practicing psychotherapist for over 20 years and I have had the privledge to know Dr. Khantzian and Dr. Albanese personally and to read their separate works. With that bias being disclosed, I highly recommend this book for anyone intersted in understanding the simple and elegant 'self medication hypothesis'(SMH) of addictive disorders. Although this phrase (coined by Dr. Khantzian over 30 years ago) is now part of our populist Zeitgist, I find it is often misunderstood and misquoted. In this volume, the authors reclaim the true meaning of the SMH and make it a usable tool to dissolve shame and empower and help clients to recover. In my own practice I find clients readily admit to their patterns of self medication and are very willing to learn and use other means to alleviate their suffering. They do not use "self medicating" as an excuse, but rather intuitively connect with how their patterns of destructive substance abuse (and process addictions)either consciously or unconscously were created and maintained. Even teenage clients grasp and use the concepts of SMH and respond to the idea that they are indeed suffering and deserve to both feel better and be freed from the destructive compulsions that have previously "worked" for them.

In this simple and very readable book, the SMH is presented in a practical and usuable manner. The authors provide empirical evidence for the SMH as a unifying theory of both etiology and treatment of the addictive disorders and the person suffering with addictive disorders is given both empathy and reverence. I have had the pleasure of hearing Dr. Khantzian teach over many years at the Harvard Medical School "Treating the Addictions" conference (This year's course can be viewed at: [...] and this volume contains many of his most wise teachings. The humility of the authors and well as their sincere caring and revernce for those suffering with addictions came through in a unique blend of theory and "how to" information.

This book now joins my list of top ten choices for training references and I hope clinical trainig programs will include it as a concise and practical text for those in both early and advanced clinical training.

This is required for SOWK grad school. It refreshingly easy to read, less jargon, I guess, is the best way to put it. A number of myths propagated by popular media and television shows are put to rest and one actually learns the real deal with AODs and SUDs.
